Minor Fishing Trip — Parent/Guardian Consent & Safety Agreement
I am the parent or legal guardian of the minor angler identified above. I authorize my child to participate in a guided freshwater fishing trip operated by Island Bass Outfitters.
1. Acknowledgment of Risks
I understand that fishing and boating involve inherent risks, including drowning, falling overboard, slips and falls, changing weather, rough water, fishing hooks, sharp equipment, wildlife, boat movement, and injuries requiring emergency assistance.
2. Safety Rules
I understand that my child must follow all instructions provided by the guide, wear a properly fitted U.S. Coast Guard-approved life jacket as directed or required by law, remain seated when instructed, and avoid unsafe behavior.
The guide may end or modify the trip when conditions or behavior present a safety concern.
3. Parent/Guardian Responsibilities
I confirm that I have legal authority to authorize my child's participation. I will provide accurate emergency contact information and disclose any safety-related needs I believe the guide should know to help accommodate my child.
4. Emergency Medical Assistance
If I cannot be reached during an emergency, I authorize Island Bass Outfitters to contact emergency services and seek reasonably necessary medical assistance for my child, to the extent permitted by law.
5. Acknowledgment and Consent
I understand the inherent risks of recreational fishing and boating and voluntarily authorize my child's participation. I acknowledge that reasonable safety precautions cannot eliminate every risk.
This agreement does not waive any rights or claims that cannot legally be waived under New York law, including any rights belonging to the minor that a parent or guardian lacks authority to release.
6. Confirmation
By signing below, I confirm that I have read and understand this agreement, am the minor's parent or legal guardian, and consent to the minor's participation in the guided fishing trip.
